1. Do you take my insurance?
All insurance must be verified by our office at least 24 hours before appointment time.
2. Are you a participating provider?
Our office accepts payment from most PPO and indemnity plans but we are not members of any “discount” provider plan. Fees are based on the experience of the provider and the cost to meet or surpass the standard of care. Our office does not discount patient care.
